Address Change

Each licensee is required to report all locations wherein he or she conducts activities related to the practice of chiropractic and/or acupuncture. Address changes for office and/or home addresses must be filed with the Board within 30 days of the change. This includes removals from places of practice.

You are prohibited from filing a post office box address without also disclosing the actual physical location of the address reported.

Address changes must be filed in writing and submitted to the Board office via email, facsimile or US Mail. Click here to download an Address Change Form.
